To join by mail, please print out a copy of the Membership Application (two pages) and the Life Insurance Beneficiary Designation Form, and remit with initiation fees and dues as outlined below.  We do not currently require any performance audition, although we request that you advise us if you are joining because you have secured employment with any local performing organizations, or hope to do so.

Membership Fees

Annual membership dues for 2007 are $122.00, and may be paid annually or semi-annually.

If you are a student (full- or part-time) or under the age of 21, you may enroll as a Student or
Youth Member, and have the $75.00 Initiation Fees waived.  You need pay only the first six months
dues of $61.00 (for 2007) and the once-a-year $10.00 life insurance premium copayment, for a
total of $71.00.

If you are joining as a regular member, and are not currently a member in good standing of any
other Local of the AFM, you will need to pay initiation fees of $75.00 plus the above-mentioned
dues and insurance premium, for a total of $146.00.  If you ARE a member of another Local,
you need only pay the Local initiation fee of $10.00, for a total of $81.00.

We are not currently able to accept credit or debit cards.  If joining by mail, please pay by check.
